Emergency Caregiver Authorization
If you are related to any child under your care but because of the unavailability of the child's parents and you need to make decisions related to the child's proper care and education, you can use the below link to open the Caregiver's Authorization Affidavit. If more permanent authority is needed or you are not related to the child, you should consider seeking a court established guardianship of the child.
